The Near North Health and Wellness Ontario Health Team is looking for residents from Nipissing and East Parry Sound to sit on their Patient and Family Advisory Committee.

The Ontario Health Team is a new model of care delivery that looks to better connect patients and providers across the district with the health services they need.

Their main goal is to improve patient outcomes by bringing services together to help people more easily navigate the local health care system.

The PFAC will work in partnership with the OHT to help ensure local priorities and health system issues are addressed collaboratively with patients and their families.

“Patients and families are at the center of our health care system and their input is vital to ensure we, as an Ontario Health Team, are making informed decisions that best meet their health and well-being needs”, said Jamie Lowery, Co-Chair of the Near North Health and Wellness Ontario Health Team.

The PFAC will consist of up to 10 members.

Members will be selected to ensure diversity that is reflective of the area’s population, in relation to age, geographic distribution, cultural diversity, socio-economic status and experience with the health system.
